To keep your Commercial Driver’s License (CDL) active, the Department of Transportation (DOT) requires a regular and recurring medical exam to ensure you can safely handle the demands of the road. What is a DOT Physical?

A DOT physical is a physical exam required by the Federal Motor Carrier Safety Administration (FMCSA). It is designed to make sure commercial drivers don’t have any physical, mental, or organic conditions that could impact their ability to safely operate a commercial vehicle.

Because safety is the priority and they have very specific requirementsm, these exams can only be performed by a certified medical examiner registered with the FMCSA.

 

Who Needs a DOT Physical?

If you operate a commercial motor vehicle across state lines (or within the state of Texas) and meet any of the following criteria, you are required to pass a DOT physical:

 

Vehicle/Job Type Criteria requiring a DOT Physical
Weight Operating a vehicle with a gross combination weight rating of 10,001 pounds or more.
Passenger Volume Transporting more than 8 passengers (including the driver) for compensation.
Public Transport Transporting more than 15 passengers (including the driver), even if not for compensation (like a church bus or shuttle).
Hazardous Materials Driving any size vehicle that transports hazardous materials requiring placards.

 

What to Bring to Your Appointment

The come prepared for a smooth appointment. This prevents delays in getting your medical card signed. Be sure to bring:

  • Your Driver’s License: A valid state driver’s license or CDL is required to verify your identity.
  • Your Glasses or Contacts: If you wear corrective lenses for driving, you must bring them for the vision test.
  • Your Hearing Aids: If you rely on them to hear properly on the road.
  • A List of Your Medications: Include the names, dosages, and the prescribing doctor’s information.
  • Medical Clearance Letters: If you have chronic conditions like diabetes, heart disease, or sleep apnea, bring recent clearance letters or compliance logs (like your CPAP usage report) from your specialist.

 

Pro Tip for the Exam: Avoid drinking heavy caffeine or energy drinks right before your appointment. High caffeine intake can temporarily spike your blood pressure, which might delay your certification.

 

What Happens During the Exam?

Our certified examiner will walk you through a standard, non-invasive checkup that takes about 30 minutes:

  1. Medical History Review: We will go over past surgeries, medications, and chronic health conditions.
  2. Vision and Hearing Checks: You must have at least 20/40 vision in each eye (with or without glasses) and be able to perceive a forced whisper from five feet away.
  3. Blood Pressure & Pulse: To ensure your cardiovascular system is stable.
  4. Physical Examination: A quick head-to-toe check of your reflexes, lungs, abdomen, and limbs.
  5. Urinalysis: This is a simple dipstick test to check for blood, sugar, or protein in your urine (which can indicate underlying conditions). Note: This is not a drug screen, though some employers may request a separate drug screen at the same time.

Once you pass, we will complete your Medical Examiner’s Certificate (Form MCSA-5876) so you can get straight to work.
