1. Photo Identification and Insurance Card

Most healthcare providers require you to bring your photo ID and a copy of your insurance card to every appointment.

2. List of Current Medications

Although most of your medications may be on file at your primary care doctor’s office, they may not have prescriptions from outside specialists.

Don’t forget to include any over-the-counter (OTC) medications, vitamins, supplements, and herbs – and the dosage and frequency. Once you make this list of prescription and OTC medications, keep it in your wallet to use at future medical appointments.

3. Relevant Medical Records or Test Results

If you’ve received a new test result since your last doctor’s appointment, bring them with you. It’s easier to remember the details if another person hears and writes down the vital medical information your doctor provides.
